SERVICE CONTRACT
This lease agreement is made between CLASSIC BOUGIE CUTE AKA' CBC, CLASSIC STUDIOS, BOUGIE HAIR & CUTE NAILS subsequently called 'OWNER; and INDEPENDENT CONTRACTOR subsequently called SELF-EMPLOYED CONTRACTOR'
As a SELF-EMPLOYED CONTRACTOR, for and in consideration of the mutual promises and stipulations contained in this agreement, OWNER does hereby co-sign you as a SELF-EMPLOYED
CONTRACTOR to CBC, and SELF-EMPLOYED CONTRACTOR hereby co-sign application use from OWNER, the contract described in Paragraph 1 upon the following terms and conditions.

And under many other physical establishments of CBC Sanitation facilities sufficient to contract and manage a SELF-EMPLOYED CONTRACTOR of an Aesthetician / Barbering / Caterer / Eyebrow & Eyelash Tech / Hair Stylist / Henna ink Artist / Linen technician / Manicurist / Tattoo; Piercing technician and Photographer. The SELF-EMPLOYED CONTRACTOR shall have the right to operate a business to the public as a self-employed professional traveling freelancer under CBC Supervision.
TERM OF AGREEMENT
This co-signing agreement shall begin today and shall continue for 1(One) Year basis until further notice between the OWNER and SELF-EMPLOYED CONTRACTOR.
After one year; the SELF-EMPLOYED CONTRACTOR can continue on a year-to-year basis; transaction rate is to be determined by OWNER or extend the lease for a year with no more than a .06% increment increase of SELF-EMPLOYED CONTRACTOR subscription fee.
After one year, owner may terminate this agreement upon a 2-month written notice and SELF-EMPLOYED CONTRACTOR can terminate with 2 months' notice after the lease term is served.

SERVICE POSITION
.
Under CBC registration a professional cosmetologist and a non-professional cosmetologist are defined as a SELF -EMPLOYED CONTRACTOR.
SELF-EMPLOYED CONTRACTORS are contracted to become a traveling freelancer.
Freelancers are defined as; any professional server who earns wages on a per-job or per-task per-offer from an establishment.
A traveling freelancers are define as; any professional server who travels and earns wages on a per-job or per-task per-offer.
A traveling freelancer is also defined as a person who offers his/her services on a self-managed basis and is not formally linked to CBC by means of a traditional full-time employment but sub-contracted as self-employed to stabilize one's independence for a secondary means of income.
SELF-EMPLOYED CONTRACTORS
are entirely responsible for one's clientele, service supplies, independently securing ones’ health benefits; transportation; business insurance, car insurance and filing taxes.
SELF-EMPLOYED CONTRACTORS are not provided with state or federal protections from agencies like the department of labor, or the equal employment opportunity commission from CBC or any businesses that contracts and book appointments with CBC freelancers.
SELF-EMPLOYED CONTRACTORS are required to be certified and possess business insurance.
A SELF-EMPLOYED CONTRACTOR shall maintain and display at their licenses and service ID to each client. Required by Federal, State and/or local authorities in the performance of providing services; failure to obtain, maintain and/or display the required license(s) will result in a loss of contract and contactor privileges and/or may be cause for immediate termination of this agreement.
A SELF-EMPLOYED CONTRACTOR shall agree to that CBC is a 3rd- party vendor that markets clients and cosmetologist and non-cosmetologist to collaborate on one platform to promote mobile services. CBC only send out notification of service to A SELF-EMPLOYED CONTRACTOR under the influence of service request. CBC is a promotion to a second means of income and service work is not guaranteed.
A SELF-EMPLOYED CONTRACTOR shall agree to that CBC is a subscription base platform that establishes sanitation facilities, not an active salon. In addition to understanding the importance of safety of both self and client from cross contamination, un-sanitized service utensils and confrontation.

RESPOSIBLITIES
A SELF-EMPLOYED CONTRACTOR shall maintain a professional appearance, effective communication, and detailed reports.
PROFESSIONAL APPERANCE:
- Clean attire must be worn; dark colored scrubs is recommended.
- Hair tied back up and out of facial area.
- Free of lose jewelry.
- Comfortable shoes.
- Service ID Badge

SERVICE RATINGS
- Profile rating is linked to contractors' profile for client’s experience.
- CBC is not responsible for low ratings that can result into no bookings.
- Each contractor is responsible for his/her customer service satisfaction, presentation, communication and appearance.
SELF-EMPLOYED CONTRACTOR will be given a contractor badge and should be worn per CBC service. Badge will consist of:
- Profile link
- Service wavier
- CBC website
- Social profile
- License display link
Any professional that contracts with CBC are subject to service clients within the following environments.
- Home Health Care Facilities
- Retirement / Retirement Facilities
- Hotels / Motels
- Group Homes
- Correctional Facilities
- Residential homes / apartments of surveillance.
- Salons needing assistance
- Events such as photo shoots, catering events, weddings.
Surveillance:
A residential property that has management or a concierge during business hours.

Non-Surveillance:
A residential property that does not have property management or a concierge during business hours.
SANITATION FACILITIES
All contractors are required to maintain service tools as clean, sanitized, bag and dated before each service appointment.

Acrylic powder (acrylic powders are acceptable if it does not carry MMA state law and used for dipping powder service only.
Any personal Service towels used are to be wash and sanitized under non-scented laundry soap.
SERVICE RESTRICTIONS
Service Refusal:
- Twisted nails.
- Overly thick nails.
- A broken nail.
- A lifted nail exposing broken skin.
- Bruised, sore, redness, injured, or visible irritation hot to the touch, bleeding nails, face or skin surface.
- Blood thinner medication – Nail enhancement restricted.
- Poor blood circulation – Mini message restricted.
Eyelashes
- Visible redness to the whitening of the eye.
- Puffy, red and swelling of the eye or near the eyelash
- Crusted lashes
- Flaking of the skin near eyelash
- Damaged or missing natural lashes that has been neglected of care
Facial, Eyebrows or Make-up application
- Severe flaky, crusted or rash to the skin that may cause bleeding
- Fungal acne or rash that may cause irritation.

Hair styling, barbering, braiding & dreads
- Severe damage to the scalp caused by flakiness, fungal or dryness.
- Over processed hair that may cause more damage.
25/25/50 SERVICE POLICY
- We value what we do; CBC cosmetologist & catering freelancer will perform 25/25/50 service.
- 25%: Contractor will discuss client service choice and suggestions.
- 25%: Contractor will stop mid-service to recap style, texture, size, color etc.
- 50%: of the remaining service will be complete.

Mid service: CBC respectfully ask for the client to tell the technician immediately that they are unhappy with the quality of the work.
In fairness, if freelancer completes a 100% service rate to only have a client acknowledge that they are not satisfied with their service will cause the client to have their account terminated. After a full investigation and refund; excluding processing, tax and traveling fees.
ACCOUNT USAGE CLIENT & CONTRACTOR
Service account cannot be in a non-active state for 69 consecutive business days.
If a service account is found non-active for 69 consecutive business days; their account will be suspended unless appeal has reconsidered circumstances.
If a service account is found non-active for 90 consecutive business days; their account will be terminated for 6 months unless appeal has reconsidered circumstances.

LOG IN / LOG OUT
All SELF-EMPLOYED CONTRACTOR shall be responsible for determining their days and hours of operation for service notification.

Each Self-Employed Contractor must sign on CBC Portal before appearing active.

Each Self-Employed Contractor must sign on CBC Driver app after logging on CBC Portal before appearing active.
PAYMENTS & SCHEDULES
Payments: All booked appointments are upfront payments.
- Review Cancellation & Refund Policy.
Scheduling: All booked services are scheduled 3 days out prior to current date.
Scheduling: All On call (immediate service) are 3 1/2 hours out.
Scheduling: Reserved appointments are scheduled 3 calendar months out.

CONTRACT RATES
Contract rate $___12.42____ due per each transaction pay out.
Contract rate is due immediately for every scheduled service accepted.
Each contractor scheduled for an appointment shall acknowledge that 12.42 will be withdrawn from each transactional payout.
All additional earning and tips from client to contractor is granted 100% payout.
Any additional service charged between the freelancer and client should be documented and signed by the client before service and payment.
Any contractor found fraudulently charging a client without proper documentation will be subjected to application termination.
ADDITIONAL SERVICE CHARGES:
Any service changes that need to be made for a booked appointment can be added or canceled no later than “1 hour before service appointment” through CBC booking profile.
Any service changes that need to be made for a booked appointment and it is “after the 1-hour service policy window” service changes are settled between the clients and a contractor.
CBC is not responsible for additional service transactions made between the client and freelancer
INSURANCE
Business insurance must be maintained active to keep service profile from suspension.
Self-employed Contractor must provide and pay for their own liability and product protection (against theft, injury, car, property damage) insurance.
Required: 3-party injury
and 3-party property damage.
Proof copy of the insurance contractor has to be provided to the CBC. Contractor should have _______________________________________ listed as a third-party email notification.
Notifications of insurance payments should be sent directly to Classic Studios service email @ info@classicstuidos.org
Any theft or damages at CBC Classic Studios location caused by a self-employed contractor or a party thereof, the damages of the property or products shall be covered by the self-employed contractor personal insurance, and the owner will not be liable for such.
SAFTEY POLICY
Field Supervisor:
- CBC- Field Supervisor is not affiliated with cities local police department.
- CBC- Field Supervisor description of service is to assist the traveling freelancer.
- CBC respectfully ask that all users respect the field supervisor's assistance to help resolve minor issues and the traveling safety of all service given.
- If any circumstances of escalation of conflict arise, please call the local Police Department.
- Non-surveillance service additional fees are $15.06.

Each contractor must be responsible, safe and possess business insurance.
CBC is not responsible for the following:
- Stolen supplies.
- Car vandalism.
- A contractor should never log off CBC driver application during service.
- CBC Driver application tracks the contractor locations.
- A contractor should always perform the 25/25/50 service recap policy.
- A contractor must have good communication and problem-solving skills.
Accidental injury to the client contractor can document incident at CBC Status & Reports profile.
A contractor is to NEVER allow the client to redirect them to a property that is not listed a service destination. CBC requires the contractor to call and confirm any changes.
POLICY ACTIONS
Excluding physical abuse; damaging property; harassment and stalking resulting a permanent profile termination.
- Must maintain business insurance.
- Must maintain service certification.
- Must maintain a 65% acceptance rate.
- Must maintain a safety, and professional service.
Violation # 1 If SELF-EMPLOYED CONTRACTOR is found in violation, written notification.
Violation # 2 if SELF-EMPLOYED CONTRACTOR is found in violation, notification of written document that is held for future violations.
Violation # 3 if SELF-EMPLOYED CONTRACTOR is found in violation, notification of written document and phone conference.
Violation # 4 if SELF-EMPLOYED CONTRACTOR is found in violation; profile suspension of 2 weeks with an appeal that determines reactivation.
Violation # 5 if SELF-EMPLOYED CONTRACTOR is found in violation; profile suspension of 2 months with an appeal that determines reactivation.
Violation # 6 if SELF-EMPLOYED CONTRACTOR is found in violation; profile termination 6 months with an appeal that determines reactivation.
Violation # 7 if SELF-EMPLOYED CONTRACTOR is found in violation; profile termination 1 year with an appeal that determines reactivation.
Violation # 8 if SELF-EMPLOYED CONTRACTOR is found in violation, profile termination with no appeal that determines reactivation.
